OptiCall: AI agents for every patient conversation

OptiCall runs patient communication for ophthalmology, LASIK, cataract, and specialty medical practices across the US and Australia. Chiles Co designed, built, and operates the AI layer behind it on Falcon Builder: voice, chat, SMS, email routing, and reporting for every practice they serve.

45+
Practice-level agents, one per client practice
600+
Published production workflows
24/7
Coverage: business hours, after hours, weekends
50k+
Patient calls handled by the AI agents every month

The challenge

Medical practices lose patients on the phone. Calls go unanswered after hours, web leads sit for days, and front desks spend their day routing messages to the right department. OptiCall's business is fixing that at scale for dozens of practices at once, each with its own locations, providers, hours, and rules.

What they needed

  • AI that answers, qualifies, and books without sounding generic
  • Practice-specific routing that staff could trust
  • Voice, chat, SMS, and email on one platform with one audit trail
  • Reporting practice managers would actually read
  • A partner who could ship new practices every week

What we delivered

An Enterprise Falcon Builder workspace with a dedicated agent for each practice, a shared library of capture, routing, messaging, and reporting workflows, and an operations layer for OptiCall's own team. Chiles Co built it and continues to run it.

What we built

Six categories of workflows, replicated and tuned for every practice in the OptiCall network.

Voice agents on ElevenLabs
Outbound call workflows that follow up with new leads, plus post-call processing that pulls transcripts and call details, extracts the outcome, and writes it back to the CRM and the practice's records.
  • Outbound lead follow-up calls
  • Post-call webhook processing
  • Call detail and transcript capture
  • Outcome extraction into GoHighLevel
Chat and SMS capture agents
Website chat and GoHighLevel conversation agents that qualify a patient inquiry, answer practice-specific questions, and drive it to a booking, a callback, or a hand-off to staff.
  • Website and GoHighLevel chat agents
  • Two-way SMS agents
  • Booking-link delivery
  • Outcome tagging: booked, callback, still researching, not reached, stop contacting
After-hours checks and department routing
Every practice gets an after-hours check and a router that sends each conversation to the right inbox: front desk, billing, surgery scheduling, technicians, accounts payable, HR, IT, or the on-call doctor.
  • After-hours detection by practice and location
  • Department email routers
  • On-call doctor text alerts
  • Medical clearance and EMR hand-offs
Patient messaging
Templated but personalized SMS and email at the moments that matter: booking confirmations, directions to the right location, pricing and financing, testimonials, and self-test links.
  • Booking confirmation SMS and email
  • Per-location directions SMS
  • Pricing, financing, and testimonial texts
  • EMR and provider notifications
Reporting
Daily and monthly reports per practice, per router, and for OptiCall's own operations, so practice managers and OptiCall leadership see what the agents did without opening a dashboard.
  • Practice daily and monthly reports
  • Weekly completion reports
  • OptiCall daily admin report
  • Monthly billing reports for US and Australia
Internal tools for OptiCall's team
Workflows that keep the platform itself healthy: a Prompt Architect chat agent that helps OptiCall staff draft new agent prompts, daily provider and location directory syncs, database backfills, patient lookup, and CRM cease-contact syncs with HubSpot.
  • Prompt Architect agent (hosted chat interface)
  • Daily provider and location sync
  • Database admin and backfill jobs
  • HubSpot and GoHighLevel sync

How the system runs

01

One agent per practice

Each client practice gets its own agent in the OptiCall workspace with its own hours, locations, providers, routing rules, and tone. Nothing is shared by accident.

02

Templates, then customization

Capture, callback, booking, reporting, and after-hours flows start from a proven template and are tuned per practice. New practices onboard in days, not weeks.

03

Human in the loop where it counts

Agents book, route, and answer. Anything clinical, billing-sensitive, or ambiguous goes to a person with the full context attached.

04

Managed, not handed off

Chiles Co runs the workspace day to day: new practices, prompt changes, new integrations, and fixes when a phone system or CRM changes underneath us.

A single lead, end to end

  1. Step 1
    Inquiry

A patient fills out a LASIK form on the practice website at 9 p.m.

  2. Step 2
    Capture

The chat or SMS agent responds within seconds, answers pricing questions, and offers a booking link.

  3. Step 3
    Voice follow-up

If they go quiet, an ElevenLabs outbound call follows up the next morning.

  4. Step 4
    Routing

Outcome is tagged in GoHighLevel. Callback requests route to the right coordinator by location.

  5. Step 5
    Report

The practice's daily report shows the booking, the source, and every touchpoint.
